Omni Consultation Forms - Completing a Form on behalf of a patient

Creating Form responses on your patients behalf

To create and complete a Form response on behalf, navigate to the Phonebar Communications Window and select the Forms tab

Once the Forms tab has opened, click the +Create New Form button at the top right hand corner to get started

Once clicked, a blank form will be opened under a new Draft Form tab

To assign a patient to the form, click the Assign Patient button

The Assign Patient modal will open. If you are currently on a call with the patient, this will search for patient records associated with the phone number. The current Active Patient will also be displayed

To assign, click the correct patient. Once selected this will turn blue, and you can click Assign to confirm

The patient will then be displayed above the form. If you need to change the patient assigned to the form, click Change Patient

You can then complete the form with the information provided to you by the patient. You will need to complete all fields that do not state optional

Once you have filled out the responses, click the dropdown for the Status, located under Action Response. Set the Status as Waiting

This will ensure that the completed form will be added to the Waiting tab ready to be actioned

To set the Priority and Severity, click the dropdown for the relevant field and choose from the following options:

  • High

  • Medium

  • Low

  • None

To assign the response to a User, click the Assign dropdown and select an individual from the list. Their availability within the Surgery Connect system is denoted by the coloured dot next to their name

Once you have assigned a response to another User, they will be notified via email

You can add any supporting comments for each response in the Comments box. Once you have entered a comment, the Cancel and Save buttons appear. Please note: comments are limited to 100 characters

If your Clinical System locks while you are creating a form it will be saved as an incomplete draft and the below warning will display

To continue, click Restore and Continue

Once you are happy with the contents of the form, click Submit New Form

If you no longer wish to continue with the new form, click Cancel to delete the response and return to the Forms tab

Once saved, the response will be saved in the Waiting tab
