Omni Consultation Forms - Form Types

Understanding the Form types available via the Omni Consultation Forms feature

By offering different Forms for your sites to use we can offer set questions relevant to patient needs, ensuring that incoming traffic can be categorised immediately and triage efficiency can be improved

We currently have three form types available for the Voice Agent and Web Form:


Medical Request Form

The Medical Request Form allows patients to provide details relevant to a new or existing medical request

When completing this request (either through the Voice Agent or Web Form), they will be presented with the following questions:

  • "Is this regarding an existing problem? Please state Yes or No"

  • "Please describe the medical problem you're calling about today"

  • "How long has this problem been going on for? Is it getting better or worse?"

  • "Have you tried anything to help the problem?"

  • "Is there anything that is particularly a worry?"

  • "Tell us in a few words how we can help"

  • "Could you please tell us the best times to contact you"

When a Medical Request form is submitted, it will pull through into he Communications Window Form Tab as Medical Request


Admin Request Form

The Admin Request Form allows patients to provide details relevant to an administrative or general enquiry

When completing this request (either through the Voice Agent or Web Form), they will be presented with the following question:

  • Briefly describe your admin enquiry

When a Admin Request form is submitted, it will pull through into he Communications Window Form Tab as Admin Request


Repeat Prescription Form

The Repeat Prescription Form allows patients to provide details relevant to their prescription/medication

When completing this request (either through the Voice Agent or Web Form), they will be presented with the following questions:

  • Name of medications

  • Is there anything else you want to tell us?

When a Repeat Prescription form is submitted, it will pull through into he Communications Window Form Tab as Repeat Prescription
